Walk-in and scheduled appointments are on Monday, Wednesday, Thursday and Friday.
To schedule an appointment, please call (preferred) or send us a message below. An OCHD representative will let you know if you need to bring anything to your appointment (immunization records, insurance cards or a physician order). You will be asked to complete a form upon your arrival and pay a fee (if applicable) for your service.
